Short Answer:
Possible reasons include:
1. You are missing some, or do not have any shipping methods setup.
2. You have excluded shipping on some/all items, at the item level. 3. You are running a promotion which discounts shipping to free.
4. You do not have weights (or enough weight) assigned to your item. This only applies to weight based methods, including real-time methods
5. You have not correctly configured your real-time shipping methods.
6. There is an error with the real-time calculation

Detailed Explanation:
1. Click on SHIPPING > All Methods. Make sure you have at least one shipping method setup. Also make sure that if you are selling to many countries (see your countries list), that you have methods setup for all of them, otherwise the shopper from that country will not be charged.
2. Click on productS > List Items. Click on a item, and look for the shipping settings. You should find an option that allows you to exclude shipping for the item. The option should not be check-marked if you want to charge shipping (this is the default behavior).
3. This discount should show up on the last step of the checkout. You could also check your promotions to ensure you are not offering any free shipping promotions.
4. If you are offering any shipping based on weight, such as range weight, or real-time, then all of your products need to have weights assigned. We have also noted that different carriers may have different rules; such as UPS seems to need at least 2 oz. Click on PRODUCTS > List Items. In the right-hand column, you will see a field where you can enter the item's weight. If you are selling items which do not need to be charged shipping, you can leave them blank.
5. Be sure that under your real-time shipping settings that you have configured them. Typically it will need settings such as how you will get your package to the carrier, zip code you will be shipping from, etc... (TAX & SHIP > Real-Time Methods > Then click [CONFIGURE] next to the method you want to edit.
6. First, do a test on the real-time shipping (in the manager there is a TEST link next to the configure link). When you do a test, it should come back with a result. If there is an error message, you may search for it in the knowledge base
7. Various carrier rules exist and are based on on your configuration, the package destination, weight, and other factors. If it seems that shipping generally is working, but occassional problems or rates do not appear, we suggest reading mroe on the related topic below.
