Short Answer:

Some Macintosh users have reported receiving a blank preview when attempting to import items. Save as CSV for Windows (or MS-DOS)

Detailed Explanation

Due to the way Mac saves the CSV file, it adds delimiters incompatible with our system (Unix).  To resolve this, when you save your CSV file on your Mac, change the format to "CSV (Windows)"
