Short Answer:

Applies only to SEO version of the cart

You need to make sure you have the .htaccess file in the same folder that the catalog resides in.

Detailed Explanation:

For example, if your catalog is in your site like this:http://www.mysite.com/elephants/catalog.html then your htaccess file would need to be in the same "elephants" folder.http://www.mysite.com/elephants/.htaccess

This also applies to any additional template files if your store uses different (outer) design templates for different categories.  You can copy and paste the .htaccess file to multiple directories if needed.
